Once again MedForce sets the standard for benefits in today’s Medical Staffing Industry by offering one of the finest and most comprehensive Health Insurance plans available. MedForce offers Blue Cross Blue Shield HMO Blue New England Enhanced Value. This is, by far, one of the best Health Insurance plans available with an over 97% Physician participation rate. MedForce helps to make your Health Insurance more affordable by paying for 50% of the cost of Individual coverage and 33% of the cost of Single Plus, Couples and Family coverage.
The following is a list of the current cost for each Employee after MedForce pays it’s percentage:
Individual coverage - $64.30 per week. Single Plus coverage - $153.51 per week. Couples Coverage - $140.57 per week. Family coverage - $225.05 per week.
Our Blue Cross Blue Shield Insurance Plan renews in February once per year. The rates listed are valid until February 2009. These rates apply to all New England employees only.
Dental Insurance
MedForce is proud to offer Delta Dental Premier High Plan with Orthodontia as part of our benefits package. We know that affordable Dental Insurance is difficult to find. Now you can have one of the most comprehensive Dental plans available. MedForce contributes 50% of the cost of Dental Insurance for both Individual and Family plans. This makes your cost just $7.25 per week for Individual coverage or $18.75 per week for Family coverage.
Our Delta Dental Premier Insurance Plan renews at the end of January once per year. These rates are valid until January 2009.
Disability Insurance
MedForce is pleased to offer a Voluntary Benefits Package from Colonial Supplemental Insurance Company. These plans supplement your current benefits to better meet the individual needs of you and your family at a very reasonable cost. We feel so strongly about the disability coverage, MedForce is paying up to $2.00 per week for each employee who participates in purchasing a disability plan. Just two or three dollars per week can pay for all of your household bills should you suddenly be unable to work.
The choices include:
Disability/Income Protection Insurance: These plans help provide an income to someone whose ability to work and earn an income is temporarily interrupted due to injury, illness or delivery of a child. These plans help pay your monthly bills such as mortgage/rent, utilities, food, car payments, insurance, phones, TV, child care/support, credit cards, etc. As you know, if you are unable to work due to an illness or injury, you would have no income to cover these types of bills. Colonial’s program offers you peace of mind should the unexpected occur. These disability plans are customized to fit your own individual/family needs and can be as affordable as one dollar per week.
Universal/Term Life Insurance: These plans are in addition to any current personal life insurance you may have. What is of great benefit is that family coverage is also available and most plans earn cash value. These policies can be purchased for just yourself, your spouse, your children or in any combination you choose.
Accident & Health: These plans pay stipulated amounts of money for various injuries and/or hospital/emergency room services/appliances. This type of insurance helps cover out of pocket expenses like deductibles, co- pays, temporary housing, specialists outside the HMO, etc. Family coverage is also available.
Cancer Insurance: The American Cancer Society has estimated that Americans spend $172 billion dollars annually for the care and treatment of cancer and that only 35% of that is covered by most medical plans. Colonials’ cancer policy can cover these out of pocket expenses such as lodging, meals, transportation, deductibles, experimental drugs, etc. The weekly cost is around $5.00.
While enrollment in these plans is voluntary, we encourage you to learn about your options. We especially encourage you to take advantage of MedForce’s benefit enhancement of paying up to $2.00 per week per employee for those who sign up for Colonials’ disability product.
Professional Malpractice Liability Insurance
We realize the importance of being heavily insured when working in today’s medical community, therefore MedForce offers FREE Professional Malpractice Liability Insurance for each and every Full Time Employee! The limits for this insurance are One Million per incident, Six Million aggregate! This is a very high amount of coverage, especially when you consider that all Employees are already covered under MedForce’s Professional Liability Insurance, but we felt that since MedForce already offers the very best of every aspect of Medical Staffing, we want our Nurses insured the most as well. Once again, MedForce sets the standard in handling the careers of Medical Professionals.
The cost of this extremely important, added protection is $104.00 per year. The Nurse must pay the $104.00 up front (or approve two paycheck deductions of $52.00 each) and once the Employee completes a full year of Full Time work with MedForce, we reimburse the entire amount. If the Nurse doesn’t quite achieve Full Time status (1,620 hours per year), but completes at least 1,152 hours per year, MedForce will reimburse half the cost of the insurance ($52.00)! This way, as long as the nurse is working at least twelve shifts per month, it will only cost them $52.00 per year to possess an unprecedented amount of Malpractice coverage!

AAA Auto Club Reimbursement
MedForce will reimburse any Full Time Employee for AAA Auto Club Standard Yearly Membership up to $60 per year for a AAA Membership that is in the Employee’s name. All any of our Full Time Employees need to do is bring in their receipt from purchasing AAA and MedForce will reimburse the full amount. Sixty dollars per year per Employee maximum.
MedForce Full Time Employee Qualifications
As always, any benefits that involve a contribution from MedForce are available only to our Full Time Employees. Enrollment for Full Time benefits is available once the Employee has maintained Full Time status for 60 consecutive days. A MedForce Employee must work, and maintain, 135 hours per month at all times to be considered Full Time.
All Full Time Employees with benefits will receive a statement in the mail every month summarizing their hours and earnings for the previous month. Employees will be able to track their hours and earnings using this report. If an Employee falls short of meeting the minimum hourly requirement for their Full Time status/benefits, MedForce will ask them to make up the time in the following month. Consistent failure to meet the hourly requirements for Full Time benefits will result in cancellation of the applicable benefits.

Part Two: Benefits available to all MedForce Employees
Weekly Payroll/Direct Deposit
MedForce pays all Employees every Friday. Any time slip(s) turned in before close of business on Tuesday will be paid on Friday. Any Employee may choose to have their pay directly deposited into one or two accounts, either checking or savings. The processing cost for each direct deposit check is $3.00. This amount will be deducted out of the Employee’s paycheck.
Referral Bonuses
MedForce offers Referral Bonuses for all of our Employees.
Each and every Registered Nurse who successfully works 500 hours within 1 year of their hire date (approximately 62 – 8 hour shifts) = $300.00 to the employee who referred them.
Each and every Licensed Practical Nurse who successfully works 500 hours within 1 year of their hire date (approximately 62 – 8 hour shifts) = $300.00 to the employee who referred them.
When a new Nurse is hired, they must name on their application the individual who referred them. MedForce will only pay one referral bonus per Nurse and only to the individual that the new employee has named on their application. If any new RN or LPN takes longer than 1 year to work the 500- hour requirement, the Referral Bonus will be forfeited. If the Nurse who referred the new Nurse to us is no longer employed with MedForce when the referred Nurse completes the 500 hour requirement, the referral bonus will not be paid. In order to collect the Referral Bonus, you must be a current MedForce employee in good standing. A MedForce employee will be considered current as long as they work a minimum of one shift per month. When bonuses are earned, they will be distributed into your next paycheck and subject to tax as any other type of income.
Vacation Pay
MedForce offers Vacation Pay for all it’s Employees. It is the Employee’s responsibility to keep all MedForce pay stubs (NOT the Employee copy of the time slip). Once the Employee has pay stubs that total 500 hours worth of time worked, they may bring the pay stubs to our office. The pay stubs will be signed off, returned to the Employee, and a vacation day will be paid at their average pay rate in the next Employee paycheck. If pay stubs are more than one year old, they are no longer valid for use towards claiming Vacation Pay. Therefore, an Employee must turn in their pay stubs within one year of receiving them in order for them to be used towards a vacation day. For example, if an Employee comes into our office on March 1st 2005 to turn in pay stubs for a vacation day, none of the pay stubs can be dated before March 1st 2004. When vacation days are earned, they will be distributed into your next paycheck and subject to tax as any other type of income.
